- notes: Printed commission form with blank fields; sole extant copy (Draper Collection at Historical Society of Wisconsin) is completed in the name of Col. William Preston of Fincastle County, then a key militia leader on the colony's western frontier; that form is dated "Williamsburg, October 7, 1775" and signed by the members of the Committee of Safety.
Form lacks printer credit; Alexander Purdie was printer to the convention that established the Committee of Safety the preceding July, so also the most likely producer of this form.
