• notes: Blank form for ordering county sheriffs to summons a witness in a law suit before a county court; appearance of only known copy – held by the Library Company of Philadelphia and acquired from the Michael Zinman Collection of Early American Imprints – suggests the form was printed three copies per page (33 cm. x 19 cm. or size of standard folio sheet).
  Form lacks identifying marks; clearly a Virginia imprint, but place and press of issue are unknown and likely unknowable; sole recorded copy has manuscript completions for a case before the Wythe County Court in July 1798, so the date assigned here to this imprint.
