Sheet lacks colophon; the Henkel Press in New Market began printing in 1806; the only copy known, held by the Historical Society of Pennsylvania, carries a manuscript note on the verso reporting: "The licentiate blank was one of the first publications of Ambrose Henkel, New Market Va, 1806, for the use of the Southern or North Carolina Synod of the Lutheran Church. Stapleton." Date, place, and press supplied here from that notation.
