- notes: Only known copy was formerly held by the Samuel Colgate American Baptist Historical Library in Rochester, New York, now a part of the American Baptist Historical Archives at Mercer University; that copy was evidently bound with the association's minutes for 1814, 1816, 1817, 1818, and 1819, a set that then suffered the loss of pages through mutilation in nearly all of those individual reports; this 1815 report lacks all after the title page.
Title page lacks imprint; the entire run of minutes found in this bound set consists of eight-page octavo reports with a title page, the minutes in 5 pages, then a letter of three pages, for eight total with a Clarksburg colophon at end of printed text; following that pattern, this item was likely printed by Gideon Butler and Alexander G. McRae, who acquired the only press there from Forbes & Alexander Britton earlier in that summer, so the attribution here.
The Early American Imprints Series filmed the one surviving page as part of the Association's minutes for 1814 (1814.073). though it was not recorded by Shaw & Shoemaker.
